Alternatively submit the form online and send fee separately. If so please include a brief description of your dog's character, breed, age etc. (Please continue on the other side if more space is needed) I enclose a cheque for Ł45 made payable to Rachel Jackson. Fee for two people fromt he same family with one dog is Ł75. Please note that this is non-refundable unless your place is taken by someone else, or the workshop is cancelled. On receipt of this booking form and fee your place will be confirmed and you will be sent further information and directions to the venue.
